SCHOOL/ SPORTS PHYSICALS
School and sports physicals should be scheduled as early as possible with your child's pediatrician. Please bring all necessary physical/sports participation forms and shot records to your appointment. There is a $10 charge for forms requested outside of the appointment date.
A copy of both the Kansas and Missouri pre-participation physical forms can be found below. Please answer all questions prior to presenting the form to the physician at your visit.
